Staff receive specialist training in dementia care, communication and behaviour support, with individual strategies in place to manage risk, reduce distress, and promote comfort.

Our team delivers 24-hour care supported by tailored activities, sensory engagement and nutrition management. We also work closely with families and community dementia teams to maintain continuity of care and promote meaningful connection wherever possible.

We accept step-down discharges from hospital and offer long-term placements for individuals requiring intensive or ongoing clinical oversight.

Care planning is multidisciplinary and responsive, with regular reviews and input from external professionals where needed. We deliver palliative and end-of-life care with a focus on symptom control, dignity, and psychological support, ensuring that residents and their families are fully supported throughout their journey.

Dee View Court offers a flexible, responsive approach to both long- and short-term complex care placements. Our specialist team is experienced in supporting adults with dementia, mental health conditions, physical disabilities and complex nursing needs.

To ensure our home is the right fit for each individual, we work closely with the person being referred, their family, and health and social care partners throughout the referral, assessment, and admission process. Our Clinical Assessors work with you to carry out an assessment promptly after receiving a referral, helping to ensure continuity of care and a smooth, well-supported transition into our service.

Referrals can be made directly to our admissions team by healthcare professionals, social workers, case managers or discharge coordinators. Once we receive the referral, we conduct a thorough pre-admission assessment, either in person or virtually, to understand the individual’s clinical, physical, and social needs. We then work with the referring team to agree on a transition plan that ensures safety, continuity and comfort.

Yes. Alderbrook is equipped to support a wide range of complex conditions, and we can provide a full care pathway, from rehabilitation and stabilisation to long-term or palliative care. Our multidisciplinary team is experienced in managing progressive conditions, and we are able to adjust the level and intensity of care as a person’s needs change over time.

We maintain clear and regular communication with all involved professionals. This includes providing timely updates, participating in reviews, and responding promptly to any concerns or changes in need. Our clinical team is always available to liaise directly with community services, GPs, specialists or commissioners to ensure joined-up care.
