We support people living with dementia, including those experiencing behaviours that may challenge, co-existing physical health issues, or increased dependency. Our approach combines person-centred care planning with consistent therapeutic routines and secure, dementia-friendly environments. Staff receive specialist training in dementia care, communication and behaviour support, with individual strategies in place to manage risk, reduce distress, and promote comfort.

Our team delivers 24-hour care supported by tailored activities, sensory engagement and nutrition management. We also work closely with families and community dementia teams to maintain continuity of care and promote meaningful connection wherever possible.

We offer structured, therapeutic care for adults living with long-term mental health conditions such as depression, bipolar disorder, schizophrenia, anxiety and personality disorders. Many of our residents have experienced placement breakdowns, delayed discharges or require stabilisation in a structured and supportive setting.

Care plans are developed in collaboration with mental health teams and include daily routines, therapeutic activities, medication oversight and behaviour support planning. Our staff are trained in de-escalation, positive behavioural support, and trauma-informed care, and we maintain close links with CMHTs and crisis teams where appropriate.

Alderbrook Care Home provides medium- to long-term care for people with physical disabilities or rehabilitation needs following illness, injury or surgery. We support residents with neurological impairments, mobility limitations, stroke recovery, MS, arthritis and other complex physical conditions.

Our service offers post-acute and maintenance rehabilitation pathways, including mobility programmes, postural management, pain management, and functional goal setting. We use assistive equipment and partner with community therapy teams to enable safe reablement or long-term support for residents with degenerative conditions or complex physical care needs.

Our nursing team provides 24/7 clinical care for residents with high-dependency needs, including PEG feeding, advanced wound care, tracheostomy management, diabetes, palliative care and multi-morbidity management. We accept step-down discharges from hospital and offer long-term placements for individuals requiring intensive or ongoing clinical oversight.

Care planning is multidisciplinary and responsive, with regular reviews and input from external professionals where needed. We deliver palliative and end-of-life care with a focus on symptom control, dignity, and psychological support, ensuring that residents and their families are fully supported throughout their journey.
