Admission

Following admission, the care team take the time to really get to know each resident and understand their wants and needs to ensure they retain as much independence as possible, whilst being always supported and treated with respect and dignity

Individual Care Plans

We take time to get to know our residents to understand their lives and loves, their interests and their triggers, to offer comfort, calm and support to live and thrive at Alderbrook.

Plans are created to meet each person’s desired outcomes, and we adapt these plans as someone’s health and needs change. We combine any medical needs with our holistic thinking around care, bringing in essential and appropriate social interactions, nutrition and physical activities to all work together to contribute to a personal care plan.

Individual Care Plans

Dementia Care

Our team are widely experienced and passionate about caring for adults living with dementia. We understand the complexities of the different subtypes of dementia. We treat every resident with respect, support their wishes and help to ease any anxiety or distress that may arise as as symptoms worsen and to ensure they always feel safe.

Mental Health Care

Mental health care is multi-layered, influencing someones behaviour, perception and cognition and includes their emotional, social, psychological and overall well-being. Our trained healthcare team and registered nurses, take the time to fully understand every single aspect that contributes to an individual’s health and put in support strategies accordingly.

Physical Disability & Rehabilitation

We also care for adults living with physical disabilities or those going through rehabilitation due to an injury or brain injury. We are here to help people every step of the way to offer support for independence and for those whose intended outcome is to return home.
